ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
Performance Acro is committed to conducting its activities in the safest manner possible. We hold the safety of the participants in the highest possible regard. However, parents must recognize that there is an inherent risk of injury when choosing to participate in recreational activities.
I hereby acknowledge that acrobatics involves a greater than normal risk of injury which could be a result from the improper conduct of the activity.
As parent/guardian, I assume all the risks and costs of my child participating in activities associated with Performance Acro and give authorisation for my child to participate in live tutorial sessions. I acknowledge and understand it is my responsibility to ensure my child is supervised and participates safely throughout the session.
I certify that my child has not been advised to not participate by a qualified medical professional and that there are no health-related reasons or problems which preclude my child’s participation in this activity.
I hereby release and agree to hold harmless and to indemnify Performance Acro employees, owners or volunteers from any claims, losses or expenses incurred on the behalf of me, my child or my child’s family.
Agreement to this Liability Waiver will act as your continued agreement to all ensuing sessions, whether in person, via Zoom or any other video conferencing tool.
